Answer

I changed a heading and now an old link is broken

Updated Table of Contents 1.8.1Tested with WordPress 7.1

If Settings → Keep old heading links working is on — it is by default — it should not be. Save the post again and check: the old anchor is recorded the next time the post is saved after the rename, not at the moment of the rename itself, so a heading renamed and never saved since has nothing recorded yet.

It is deliberately cautious. A heading tidied, reworded or extended is matched and its old link kept; a heading replaced with a different one is not, because pointing an old link at unrelated words sends the reader somewhere they were not going. If two headings could each be the rename, neither is chosen.

For a heading you know in advance is important, an explicit id in the editor's Advanced panel is still the certain answer. Settings → Keep existing heading IDs is on by default, so the plugin will use yours and never rewrite it.
